      The JCCEO-Jefferson County Cares Household Support Program provides assistance for individuals and households that are directly impacted by a loss/reduction of income due to COVID-19. The services include assistance with outstanding rent or utility balance incurred during the eligibility period of

      beginning March 1, 2020, and ending December 30th, 202.

       

       

    • INSTRUCTIONS:

      1. Complete application packet.
      2. Include COPIES of required documents. (DO NOT INCLUDE ORIGINAL DOCUMENTS)
      3. Place documents inside the large white envelope that has been provided for you.
      4. Be sure to seal the envelope.
      5. Drop in the designated Drop Box located at our headquarters location.

      A CLIENT MAY BE ELIGIBLE FOR ASSISTANCE FROM JCCEO AS A RESULT OF THE FOLLOWING SITUATIONS:

      1. Temporary loss of income due to Covid-19.
      2. Reduction in work hours or reduced pay due to Covid-19 (PROOF REQUIRED)
      3. If you are on a fixed income such as social security or SSI, YOU ARE NOT ELIGIBLE for this program but you may be eligible for other funding. For more info, please email us at csbg@jcceo.org

      REQUIRED DOCUMENTS:

      1. Completed Application – Release of Information and Duplication of Benefits Certification
      2. Proof of residency in Jefferson County-Lease, Rental Agreement, etc.
      3. Written documentation/bill showing amount due and dates for the outstanding balance
      4. Documentation of Covid-19 Impact – Letter from employer, pay stubs, unemployment, or other documents that demonstrate a loss or reduction due to Covid-19
      5. Picture ID, Driver’s License, and Social Security Card for yourself and all household members
      TO DIGITALLY SIGN THE APPLICATION, YOU MUST OPEN THE APPLICATION IN A PDF READER. (Adobe Reader, Foxit PDF Etc.)
