Crisis Assistance: Your original doctor's statement is required for all crisis assistance (if you have a health condition that will be aggravated if an interruption of service occurs). Some exceptions will be considered for children five and under and individuals aged 60 and above.
A PHYSICAL (PAPER) COPY OF EACH REQUESTED DOCUMENT MUST BE PRESENTED AT THE TIME OF APPOINTMENT. IMAGES ON ELECTRONIC DEVICES ARE UNACCEPTABLE.
Non-biological parents must provide custodial or legal guardianship papers.
